This quote is a play on words that refers to the fact that people who stand on their heads in the urinal will end up with wet feet. The person who stood on his head in the urinal was not alive because he did not have the smarts to get back up. The one who did get back up, however, was still alive although he did not have a dry foot or hand. This beautifully illustrates that the one who does not move forward is dead while the one who does, is still alive.
